Bone grafting, whether horizontal or vertical, has become a cornerstone of modern dentistry. This lecture will explore proven techniques for bone augmentation, including guided bone regeneration using diverse materials and bone replacement grafts, as well as mucosal detachment for optimal tissue adaptation. A key focus will be on the essential principles required for successful bone augmentation, summarized by the PASS principle: Primary wound closure, Angiogenesis, Space maintenance, and Stability.
This module will explore the latest biological and mechanical insights into the Socket Shield Technique, providing a step-by-step clinical procedure and showcasing long-term clinical cases.
Additionally, the approach to vertical bone augmentation must be reconsidered in light of patient expectations. Many patients currently face treatment times of up to two years, undergoing four to five surgeries. In this context, we believe these concepts should be critically re-evaluated:
- Are there possibilities to shorten treatment times?
- Can we reduce the number of surgical interventions without compromising outcomes?
New strategies to address these challenges will be presented, alongside advancements in soft tissue management for vertical augmentation, with a special focus on Patient-Reported Outcome Measures (PROMs). Given the importance of extensive soft tissue mobilization for vertical augmentation, we will also discuss ways to minimize morbidity and postoperative swelling, ensuring a more predictable and patient-friendly approach.
Following tooth extraction, a well-described process of wound healing results in an extensive three-dimensional bone resorption of the alveolar ridge that present a significant problem for later prosthetic reconstruction either with a conventional bridge or an implant-supported crown. Modern diagnostic techniques and scientific evidence allow the reconstructive dentist to estimate the amount of post extraction resorption and select the most appropriate clinical management for the post extraction site including immediate implant placement, ridge preservation with delayed implant placement or delayed implant placement with simultaneous lateral augmentation.
